    1. Indoors (on what floor) or in open air?

    4. Give a shortdescription [sic] of the topography.
  6. Time and character of the shock.
    1. How many shocks were felt?

  8. Intensity of the shocks.
    1. What number do you estimate for the intensity of each shock according to the Rossi-Forel Scale?
    2. How do you form this estimate?
    3. If buildings were damaged state nature of damages and the character of the buildings.                      
      I.  Perceptible, only by delicate instruments. 
      II.  Very slight, shocks noticed by few persons at rest. 
      III.  Slight shocks, of which duration and direction was noted by a number of persons. 
      IV.  Moderate shock, reported by persons in motion; shaking of moveable objects; cracking of ceilings. 
      V.  Smart shock, generally felt; furniture shaken; some clocks stopped; some sleepers awakened. 
      VI.  Severe shock, general awakening of sleepers; stopping of clocks; some window glass broken. [End Page 2] 
      VII.  Violent shock, overturning loose objects; falling of plaster; striking of church bells; some chimneys fall. 
      VIII.  Fall of chimneys; cracks in the walls of buildings. 
      IX.  Partial or total destruction of some buildings. 
      X.  Great disasters; overturning of rocks; fissures in the surface of the earth; mountain slides.

  10. Sound Phenomena.
    1. Was any sound heard at the time of the earthquake? If so describe its character?
    2. Did it precede, coincide with, or follow, the shock?
  11. Note any other phenomena which may be related to the earthquake, such as effects on anumals [sic], on springs or streams, displacements of the soil, meteorological changes, etc.
  12. What reports have you heard from your general neighborhood?
  13. Please forward local newspaper clippings relative to the shocks?
