It closes with a note, "I am appealing to your patriotic sense of duty to furnish on the enclosed blank the information requested." The document is typesigned "R.C.L. Moncure, Collector" and the head of document reads: "Treasury Department, Internal Revenue Service, Richmond--Virginia."
