Description
Patient case reports for St. Luke's Hospital, in London, from 1839-1840 were completed in manuscript by James Luke on pre-printed forms which were bound as a ledger. The printed headings are: Name -- Age -- Residence -- Occupation -- Married or not -- Habits -- Religion -- When Attack commenced -- 1st Attack or not -- Supposed cause -- Relatives Affected -- Evidences of patient's Insanity -- Dangerous to self or others -- What variation -- Has Patient been under treatment, and how long -- State of Bodily Health -- Admitted -- Discharged -- How.
